Components of a Nursing Neurological Examination

A comprehensive neurological nursing assessment includes neurological observations with the use of various scales, growth and development, including fine and gross motor skills, sensory function, seizures and any other observations. A nurse should include the following points during neurological assessment:

  • Assess level of consciousness
  • Arm and leg movements, assess both right and left limbs and document any differences
  • Pupil size, shape and reaction to light
  • Vital sign changes

At the end of this topic, a nurse will be able to demonstrate techniques to:

  • Determine mental status of a patient
  • Obtain a Glasgow Coma Scale
  • Assess the pupils
  • Assess motor function
